Screening Result (PQ-1)
The result of screening candidate contractors with a one-page survey and 11 public-record checks. Item-by-item findings for self-reported items (60 points) and public records (40 points), 5 gates, and a recommendation (advance · hold · exclude).
Verification Report (PQ-2)
Grade (A–D), contract limit and contract conditions determined from documents, public-record cross-checks and a survey of the references the contractor designates. 2-page summary + details (general · track record · finances · reputation · gaps).
Performance Review (PQ-3)
The post-award performance review of the awarded contractor. Six dimensions — owner assessment, contract performance, safety, timely pay to subcontractors and suppliers, workforce stability, financial update — and a grade update.
Reports are opinions based on the public records, submitted materials and surveys available at the time of the inquiry and do not guarantee contract performance. Contractors under verification do not receive scores or grades — only the items to correct.
